Hammer Rest. Scholar Athlete Nomination Form
The Hammer Restoration, Inc. Scholar Athlete of the Month will be awarded to one local high school student per month.
Winners can add this honor to college applications and will be announced during a live sports broadcast on ESPN 100.9 FM.
Two students, one male and one female, will be chosen each month for athletic and academic achievements as well as their leadership skills.
Nominations forms may only be submitted by coaches, ADs, teachers or school administrators.
